It's kind of a meme, and kind of an inside joke.
Basically Drew Barrymore has a show where she regularly behaves in a strange way towards her guests.
The still-shot is from her interview with Christina Aguilera, and she looked particularly psychotic and depraved so I made it into a horror movie poster.

For context, she was a child actor who appeared in Steven Spielberg's move E.T. The Extra-Terrestrial.
Hollywood has a reputation of the abuse of young actors, and there's always speculations/accusations.
